The Public Health Service Grant Application (PHS 398) is used to apply for training grants. Please note that special instructions for training grant applications are included in section V of the PHS 398 booklet, and that substitute table of contents and budget form pages are provided.
NIH Institutes and Centers may have special policies and requirements for their T32s, and may provide sample tables and suggested formats for the information to be included in the program plan. Therefore, applicants should contact the prospective awarding component (training grant contact people for each institute are listed at the end of the Program Announcement).
